Community Dental Program

The Community Dental Program exists to assist income qualified individuals access needed dental services.

All Dental services must be pre-approved, and once approved can provide up to $2500 per person or 2750 per household toward dental services with participating providers.

Programs are subject to income qualification limits that are 200% of the Federal Poverty level and must be pre-approved prior to accessing services.

Colorado Dental Health Care Program for Low Income Seniors

Only approved CDT codes are provided under this program. Implants are NOT covered.

Are you able to pay the associated co-pays with the procedures, and are you willing to pay those directly to the Dentist of your choice, off our providers list at the time of services rendered?    

 

To Qualify for this program, you must be sixty years of age or older, you cannot have DentaQuest or any other form of dental insurance. Your household income must also be at or below 250% of the current years’ federal poverty level.
 

Gross Income must be verified for the entire household by providing any of the following documentation: SSI Statement, Pay Stub, Annuity statement, and/or current tax documents. A bank statement is not required but may be requested to verify any other forms of income.  A current driver’s license can be used to show proof of residency and age but must be current. All CICP clients must have a current card.
